Please bring the all important Emergency Information Form to Registration in July. This form is necessary in order for me to treat your student, should it be needed. There is also a section for pertinent health information as well as emergency contact information.
Please bring any new immunizations, or the entire record, to registration as well. I just love to add these to your student’s records.
